Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Seniors Resource Center (SRC) is looking for a Nurse to support our on-site Adult Day Services. Our Adult Day Program is open Monday through Friday and provides participants with daily socialization, activities, light breakfast, and lunch in a safe environment. We are looking for individuals who have a heart for older adults and are compassionate, reliable, dependable, and friendly.
SRC is dedicated to enhancing the independence, dignity and quality of life for seniors in the Metro Denver Area and surrounding communities. We believe seniors are a vital part of our community. Services such as in-home care and adult day programs provide life-enriching activities and support both older adults and their caregivers. We help older adults remain independent and engaged.

Job Type: Full time (M-F) Days 40 hours
Starting Pay: $28.00 – $30.00/hour, DOE

Brief Overview:
The Nurse is responsible for monitoring the general health of Adult Day Program participants, providing basic first aid as needed, medication management and care planning for each client. This is not a typical nurse job, it’s a hands on opportunity to interact with participants and their families regularly. The Nurse works closing with the Care Manger and Adult Day team to ensure that the participant and their caregiver have support.
What you will do:
- Write individualized participant care plans and biographies including information about a participant’s needs and abilities in relation to activities of daily living (ADLS), psychosocial, dietary, medications
- Communicating with physicians for orders, and work with team as needed to obtain needed documents, such as advance directives, vaccination records, etc. in accordance with center policies and protocols
- Identify realistic goals for participants while at the program
- Communicate to interdisciplinary team how to best support participant in achieving those goals
- Communicating changes in condition, pain, concerns to care partners and documenting that communication in medical record
- Communicating with physician offices about medications needed to be taken on site, sending and receiving orders
- Conduct participant assessments
- Coordinate with family and any outside additional help, such as PT, OT
- Assist in the ongoing oversight, training and communication of QMAPS, CNAs) and other program staff that assist with participant ADL care
- Monitor and assist with activity programs and mealtime as needed to assess, participant changes in functional ability and behaviors provide intervention as needed
- Lead educational meetings and trainings to staff
- Facilitate support group for Caregivers
- Partners with site manager on other duties and leadership responsibilities
Required Qualifications:
- Dependable and committed to improving older adults’ lives
- Strong time management skills
- Patience and understanding of the older adult community
- Excellent interpersonal, organizational, and communication skills
- Valid Registered Nurse or Licensed Practical Nurse License in the State of Colorado
- Experience working with individuals with Dementia
- Smartphone and willingness to learn/use technology
- Must be vaccinated or willing to be vaccinated to start - SRC is federally funded and required to have all employees vaccinated – this includes first two vaccinations and a booster, or one dose of a single dose vaccine and a booster
- Able to successfully pass a criminal background check
Desired Qualifications:
- Current Basic Cardiac Life Support (BCLS) card or the ability to obtain one upon employment
- Experience in a care setting for the frail and cognitively impaired elderly
- Comprehensive office experience, including excel, electronic faxes, emails, answering phones, making copies etc. or ability to learn
