Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Seniors Resource Center (SRC) is dedicated to enhancing the independence, dignity and quality of life for seniors throughout the Metro Denver Area and surrounding communities. Seniors Resource Center is looking for a nurse to support our on-site Adult Day Respite Services (ADRS). Our adult day program is open Monday through Friday and provides participants with daily socialization, activities, light breakfast, and lunch in a safe environment. We are looking for individuals who have a heart for older adults and are compassionate, reliable, and friendly.

Job Type: Full time (M-F) Days 40 hours
Starting Pay: $30.00 – $38.00/hour, DOE

Brief Overview:
This is not a typical nursing role; SRC’s adult program is grounded in a social model of care rather than a clinical one. The position provides regular, relationship-centered interactions with participants and their families, with an emphasis on enhancing quality of life. The ADRS Nurse works in close partnership with SRC’s Care Management and ADRS teams, offering guidance and support to help ensure team members feel supported and successful.
What you will do:
- Write individualized participant care plans and biographies including information about a participant’s needs and abilities in relation to activities of daily living (ADLs), psychosocial, dietary, medication administration
- Communicate with physicians for orders, and work with team as needed to obtain needed documents, such as advance directives, vaccination records, etc.
- Identify realistic goals for participants while at the program
- Communicate to interdisciplinary team how to best support participant in achieving those goals
- Communicate changes in condition, pain, concerns to care partners and documenting that communication in participant record
- Communicate with physician offices about medications requiring onsite administration, send and receive orders
- Conduct participant assessments
- Coordinate with family and onsite therapy providers, as need for therapy arises
- Assist in the ongoing oversight, training and communication of QMAPS, CNAs and other program staff that assist with participant ADL care
- Monitor and assist with activity programs and mealtimes, as needed, to assess participant changes in functional ability and behaviors, providing intervention as needed
- Lead educational meetings and trainings to staff
- Facilitate support group for caregivers
- Partner with site manager on other duties and leadership responsibilities
Required Qualifications:
- Dependable and committed to improving the lives of older adults
- Strong time management skills
- Patience and understanding of the older adult community
- Familiarity with Alzheimer's Disease, and communication skills
- Excellent interpersonal, organizational, and communication skills
- Valid Registered Nurse or Licensed Practical Nurse License in the State of Colorado
- Smartphone and willingness to learn/use technology
- Must be vaccinated or willing to be vaccinated to start
- Able to successfully pass a criminal background check
Desired Qualifications:
- Current Basic Cardiac Life Support (BCLS) card or the ability to obtain one upon employment
- Experience in a care setting for the frail and cognitively impaired older adults
- Comprehensive office experience, including Excel, electronic faxes, emails, answering phones, making copies etc. or ability to learn
